Terms And Conditions For Chiropractic Services By Synergy Sports & Spine
These Terms and Conditions (“Agreement”) govern the provision of chiropractic services by Synergy Sports & Spine (“Provider”) to clients (“Client” or “You”). By receiving chiropractic services from Synergy Sports & Spine, you acknowledge and agree to the following terms:
Scope of Services: Synergy Sports & Spine offers chiropractic care, which includes assessment, diagnosis, and treatment of musculoskeletal conditions, with a focus on spinal health. The services provided may involve spinal adjustments, soft tissue therapies, rehabilitative exercises, and lifestyle recommendations.
Professional Relationship: The chiropractic care provided by Synergy Sports & Spine is based on a professional practitioner-client relationship. The Provider will make every effort to provide accurate and effective care in line with established standards of practice.
Assessment and Consent: Prior to receiving any chiropractic treatment, the Provider will perform a thorough assessment, which may include reviewing medical history, physical examination, and diagnostic imaging. The Client’s informed consent will be obtained before any treatment is administered.
Treatment Plan: Based on the assessment, the Provider will create a personalized treatment plan tailored to the Client’s specific needs and goals. The treatment plan may involve a series of sessions, and the Client’s progress will be monitored and adjusted as necessary.
Risks and Benefits: Chiropractic care, including spinal adjustments, carries inherent risks. While the Provider aims to minimize risks, there may be potential adverse effects. It’s important to discuss any concerns or pre-existing health conditions with the Provider before starting treatment.
Confidentiality: All personal and medical information shared with Synergy Sports & Spine will be treated as confidential, in accordance with applicable privacy laws and regulations.
Payment and Fees: The Client agrees to pay the fees associated with the chiropractic services provided by Synergy Sports & Spine as outlined in the payment agreement. Payment is expected at the time of service unless alternate arrangements have been made.
Cancellations and Rescheduling: Clients are requested to provide notice of cancellation or rescheduling at least 24 hours before the scheduled appointment. Failure to do so may result in a cancellation fee.
Client Responsibility: The Client agrees to provide accurate and complete medical history information, follow the Provider’s recommendations, and communicate any changes in health or discomfort experienced during treatment.
Termination of Services: The Provider reserves the right to terminate or discontinue services if the Client’s behavior or circumstances pose a risk to the Client, the Provider, or other clients, or if the Provider determines that chiropractic care is not appropriate.
Liability and Indemnification: Synergy Sports & Spine and its practitioners will not be held liable for any adverse effects, injuries, or damages that may arise from the chiropractic services, except in cases of negligence. The Client agrees to indemnify and hold harmless Synergy Sports & Spine from any claims arising out of the services provided.
Governing Law and Jurisdiction: This Agreement is governed by the laws of the jurisdiction in which Synergy Sports & Spine operates. Any disputes arising from this Agreement will be subject to the exclusive jurisdiction of the courts in that jurisdiction.
